Policies

Scheduling

We require an active credit or debit card to be on file prior to scheduling. In addition, an intake packet must be completed and submitted for our provider to review prior to scheduling.

Please note that the act of scheduling an initial evaluation does not create the client/provider relationship. That relationship is developed during the appointment once the provider and client are in agreement to move forward.

Late Cancellation, Reschedule, and No-Show Fee

We request the client provide us with at least a 24-hour notice if you need to cancel or reschedule your appointment. No-show/late cancellations will result in a fee that is the full self pay rate for the appointment. This is not paid by insurance and will be charged to the card on file at the time of the scheduled appointment. Please note that this fee is not paid by insurance companies and will be charged to the card on file. Two or more missed/late cancellation appointments may result in dismissal from our office.

Late Arrivals

At Pathways Mental Health Services we take pride in providing quality care to our clients. We have extended our appointment times to allow for adequate evaluation, education, and treatment. We ask you arrive and are in the waiting room 5 to 10 minutes prior to your scheduled time. Our provider wants you to get the most out of the time alotted to you and makes every effort to stay on schedule out of respect for your time.

Our office is unable to ensure you will be seen if you arrive late.  This will be considered a late cancellation/reschedule and is subject to the full rate of the appointment. This fee is not billed to the insurance and is the responsibility of the client. It will be charged to the card on file at the time of the appointment.

Paperwork

All requested paperwork is completed outside of the scheduled appointment and is at the discretion of the provider. The $25 to $125 fee is dependent on the time (15 minute increments) needed to complete the paperwork.

The fee is not billed to the insurance and is the responsibilty of the client.

​

The provider will have 10 working days to complete the paperwork.

​

Clients must be established for 6+ months for any paperwork other than IEP or 504 plans for school. 

​

Our practice does not provide paperwork for emotional support animals.

 

Our office does not assist with long-term disability applications however, clients are able to request a copy of their medical records to assist with their long-term disability application.

Telehealth Policies

Client agrees to be in a quiet private room during all virtual encounters for HIPAA and privacy reasons. Provider should be made aware of anyone attending the appointment with the client.

Client agrees to never log in or answer virtual appointments while driving. If driving or in a moving car, provider will terminate the call and the visit may be subject to the $75 late cancellation/no show fee.

Client MUST be in the state of TENNESSEE to complete virtual visits due to licensing restrictions, as Crystal Lane, MSN, APRN, PMHNP-BC is ONLY licensed in the state of TENNESSEE and cannot legally practice outside of the state of TN.  Client agrees should they be out of state during scheduled appointment, they will reschedule no later than 24 hours prior to the appointment. Failure to do so will result in the full rate of the appointment and will be charged to the card on file at the time of the appointment.

Client agrees to be of presentable dress during virtual visits  just as if they were in the office.  If the client is observed to be dressed inappropriate, sleeping in bed, using illicit substances, or using profanities the provider will end the appointment. The client will be responsible for the full rate of the appointment. 

Controlled Substances

For your protection and safety, Pathways Mental Health Services follows federal and state prescribing guidelines for safely prescribing controlled medications.  Examples of these medications are Adderall, Jornay PM, Ativan, and Klonopin. This practice does not prescribe Xanax.

Controlled medications are prescribed judiciously and are not guaranteed to be prescribed at the client’s initial visit or any visit thereafter.

​

Psychological testing may be required to support an ADHD diagnosis prior to any stimulants being prescribed.

​

No controlled substances will be prescribed to clients who are seen exclusively by telehealth. Anyone receiving controlled substances, is expected to be seen in person for the initial evaluation and for 50% of all following appointments. 

​

Your previous medical records must be received and reviewed prior to being prescribed a controlled medication. The client can make a request with the previous provider to have them sent to our office for review prior to the initial evaluation.

​

A controlled medication agreement must be agreed upon and signed by the client before being prescribed a controlled substance. The agreement allows for random urine drug screening and pill counts.  This is an agreement that controlled medications will not be self-adjusted by the client, will not be filled early, and provider will not replace if lost or stolen. A violation of this agreement will result in dismissal from the practice.

Refills

Established patients will request refills by text via the Spruce App. Refill requests between appointments require a $25 fee be paid prior to the refill request being processed. In addition, a follow-up appointment must be made. Enough medication to bridge the gap until your appointment will be sent in. The only exception is if the request is determined to be the result of an error on the provider's part. No fee will be charged in this instance.

 

We request that you first contact the pharmacy to ensure you do not have a refill on file (even if the bottle says "No refills"). Only send one request per medication and allow 3 business days for the medication to be sent in. Our business days are considered Wednesday through Saturday.  Include your name, date of birth, medication (including dose and how often you take it), pharmacy including phone number and address.

Please note, that no stimulants will be filled between appointments.
